After killing the rats in the previous quest, return to the village and the old man will come and talk to you, claim that quest to receive the HP Potion Schemes in full and another quest will pop up asking to craft one.
Dialogue:
Hello _____! I heard about your first victories in the fortress. I must admit I'm really surprised at how well you are going.
If you ever get tired or wounded, I've something for this. Take this potion recipe. Use potion to get your HP back.
Also, I need a favor as my friend is ill. Please prepare me 1 or 2 vials of it, ok? See you soon!
